Report Personal Accident Claim
- Immediately report your claim to Philippine British Assurance Co., Inc.
- Submit all REQUIRED DOCUMENTS either to the Home Office, branch or agency where acquired your insurance.
- The outlet where your filed your claim will inform you of the approved amount of claim.
- Photocopy of Insurance Policy
- Photocopy of Official Receipt (OR) of premium payment
- Accident Report form
- Original copy of Medical Certificate
- Original copy of medical receipts and its prescription
- Statement of Account issued by the hospital
- Result of X-ray, CT Scan and laboratories
